Position Overview

We are pleased to present an opportunity to become a part of our Early Intervention Community Team at Birmingham Community Healthcare NHS Foundation Trust, a leading organization in community health services.

This role is integral to our Early Intervention initiative, which has significantly improved patient care and resource management in the past year.

Key Responsibilities

The successful candidate will collaborate with a multidisciplinary team to assist patients in their recovery journey, aiming to enhance their independence within the framework of the Early Intervention Community Team (EICT).

Responsibilities include:

  • Supporting clinicians in implementing rehabilitation strategies post-assessment, either in the patient's residence or in community settings.
  • Working flexible shifts across a 7-day schedule to meet service needs.

Job Duties

The role requires the ability to work autonomously at times, planning and executing interventions while maintaining regular communication with designated clinicians who will oversee patient assessments and rehabilitation plans.

The post holder will focus on promoting patient autonomy while adhering to established rehabilitation protocols.

Candidate Profile

Qualifications and Training

Essential Requirements

  • Minimum of 5 GCSEs including English Language or equivalent qualifications.
  • Proven proficiency in English, both written and spoken.
  • NVQ Level 3 in care or a comparable qualification.
  • Evidence of ongoing professional development.

Experience

Essential Requirements

  • Experience in providing care for older or disabled adults in community or inpatient environments.
  • Prior experience working with adult populations.

Skills and Knowledge

  • Strong communication skills for conveying complex information effectively.
  • Practical knowledge of various therapeutic techniques and procedures.
  • Willingness to participate in relevant training.
  • Ability to manage a diverse range of tasks and learn new clinical procedures as necessary.
