Position Overview

We are pleased to present an opportunity to become a vital member of our Early Intervention Community Team at Birmingham Community Healthcare NHS Foundation Trust, recognized as the largest community trust in the nation.

This role is part of a comprehensive Early Intervention initiative that has significantly reduced the demand on acute services, leading to substantial savings that can be reinvested into health and social care improvements.

Our collaborative approach involves six health and social care organizations working together to provide an integrated service. Our partners include local councils and mental health trusts, all dedicated to enhancing community health outcomes.

Key Responsibilities

As a Rehabilitation Support Specialist, you will engage with patients as a member of a multidisciplinary team, assisting them in their recovery journey and promoting their independence within the framework of the Early Intervention Community Team (EICT).

Your role will involve supporting clinicians in implementing rehabilitation plans, either in the patients' homes or community settings.

You will be required to work flexible shifts throughout the week to meet service demands.

Job Duties

In this role, you will work autonomously at times, planning and delivering interventions while maintaining regular communication with designated clinicians who will oversee patient assessments and rehabilitation strategies.

Your focus will be on fostering patient independence while adhering to the established rehabilitation protocols.

Qualifications and Experience

Essential Qualifications

  • Minimum of five GCSEs, including English Language or equivalent.
  • Proven proficiency in English, both written and spoken.
  • NVQ Level 3 in care or an equivalent qualification.
  • Demonstrated commitment to ongoing professional development.

Required Experience

  • Experience in caring for older or disabled adults in community or inpatient settings.
  • Prior experience working with adult populations.

Skills and Knowledge

  • Theoretical and practical understanding of various therapeutic methods.
