As a member of the Early Intervention Community Team, you will contribute to a significant program that has achieved remarkable outcomes, including a substantial reduction in acute and non-acute bed usage, ultimately saving millions for reinvestment in health and social care.
This role involves collaboration with a multidisciplinary team to assist patients in their rehabilitation journey, aiming to maximize their independence within the framework of the Early Intervention Community Team (EICT).
The successful candidate will support clinicians in implementing rehabilitation strategies, either in the comfort of the patient's home or in community settings.
Flexibility is essential, as the position requires shift work across a 7-day week to meet service demands.
Key Responsibilities:
- Regularly check your Trac account for updates regarding the shortlisting and selection process.
- Complete all sections of the application form thoroughly, ensuring that referee details are accurate and comprehensive.
- Be aware that the recruitment process utilizes a third-party system (TRAC) for application management and communication.
The role requires the ability to work autonomously at times, planning and delivering interventions effectively. You will maintain consistent communication with designated clinicians who will evaluate patients and provide tailored rehabilitation programs.
Promoting patient independence while adhering to the rehabilitation plan is a core aspect of this position.
