About the Role:

We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in providing high-quality care and support to our residents at Hunters Moor in Birmingham.

Key Responsibilities:

  • Support residents with their daily living needs, including personal care and health requirements.
  • Work closely with the Multi-Disciplinary team to deliver timely and effective rehabilitation plans.
  • Empower residents to be as independent as possible by providing emotional support and promoting their autonomy.
  • Monitor and report on resident progress, contributing to the development of individualized care plans.
  • Participate in decision-making processes to ensure holistic management of residents' care.

Requirements:

  • Previous experience in a healthcare setting is desirable, but not essential as training is provided.
  • A genuine passion for delivering person-centered care and a willingness to learn and develop.
  • Ability to work effectively as part of a team and communicate with residents, families, and colleagues.
  • Successful candidates will be required to undergo an Enhanced DBS check.
