If you possess experience in community nursing and are passionate about enhancing the quality of life for patients with progressive, life-limiting conditions, we invite you to explore this opportunity.
This role offers a unique chance to be part of a dedicated community specialist palliative care team. You will play a vital role in providing compassionate care and support to individuals and their families during challenging times.
We seek a proactive and experienced nurse to join our Palliative Care Team. While prior experience in community settings is preferred, we welcome all qualified applicants.
Key Responsibilities
As a community clinical practitioner, you will utilize your theoretical and practical expertise in palliative care to:
- Deliver specialized nursing care, offering advice and guidance to patients and families.
- Assess, plan, implement, and evaluate evidence-based nursing interventions.
- Provide expert clinical advice and work autonomously while managing your own caseload.
- Offer specialized education and training to colleagues and support staff.
- Collaborate effectively with clinical teams to ensure comprehensive patient care.
- Uphold the values of the Health Board and encourage others to do the same.
Proficiency in Welsh is desirable for this position; however, applications from both English and Welsh speakers are encouraged.

Qualifications and Experience
Essential Qualifications
- Registered Nurse (Part 1) with NMC registration.
- Bachelor's Degree in a relevant field.
- Post-registration qualification in a specialist area or equivalent experience.
- Knowledge of service improvement methodologies.
- Ability to conduct literature searches and critically evaluate research.
Essential Experience
- Relevant experience at Band 5 level.
- Experience in the relevant specialty.
- Background in a multi-professional environment.
- Mentorship experience with pre-registration nursing/midwifery students.
- Experience in conducting audits.
Language Skills
Desirable
- Welsh Speaker (Level 1).
