Community Nurse Specialist

An exciting opportunity has arisen for a skilled and dedicated nurse to join our Community Team for People with a Learning Disability in Hywel Dda University Health Board. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, evidence-based nursing care to adults and young people with a learning disability.
Main ResponsibilitiesYou will work collaboratively with colleagues, families, and carers to provide specialist advice, support, and training. Your expertise will be invaluable in helping us improve service delivery and promote positive change in healthcare. Person SpecificationWe are looking for a registered nurse with a good understanding of current policy and practice in relation to people with learning disabilities. Essential qualifications include a current NMC registration, and desirable qualifications include a teaching qualification or equivalent. Experience of working with adults with a learning disability is essential, and experience working with community LD teams and primary care services is desirable. A Welsh speaker is also desirable.
