Community Nurse Specialist

An exciting opportunity has arisen for a skilled and dedicated nurse to join our Community Team for People with a Learning Disability in Hywel Dda University Health Board. As a Community Nurse, you will play a vital role in delivering high-quality, evidence-based nursing care to adults and young people with a learning disability, working in partnership with them and their families.
Main ResponsibilitiesYou will be expected to work proactively and creatively, using your skills and experience as part of a multidisciplinary team to deliver high-quality nursing care. This will involve providing specialist advice, support, and training to colleagues, families, and carers, as well as developing your own clinical competency profile and skills in specialist areas.
You will also have the opportunity to work closely with other colleagues in the multi-disciplinary team, providing regular supervision, professional development, and access to peer nursing support. We are keen to support nurses who are willing to share new ideas and suggest improvements, and we have close links with Improvement Cymru and access to dedicated in-house project management support.

Our 11,000 members of staff provide primary, community, in-hospital, mental health, and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.
We do this in partnership with our three local authorities and public, private, and third sector colleagues, including our volunteers, through four main hospitals, seven community hospitals, 48 general practices, 99 community pharmacies, and numerous locations providing mental health and learning disabilities services.
Person SpecificationWe are looking for a Registered Nurse (RNLD, RGN, or RMN) with a current NMC registration and a good understanding of current policy and practice in relation to people with Learning Disabilities. Desirable qualifications include a Registered Nurse Learning Disabilities (RNLD) and a Teaching Qualification or equivalent, or recent experience in the provision of training to health staff.
Experience of working with adults with a learning disability is essential, and experience working with community LD teams and Primary care services is desirable. The ability to speak Welsh is also desirable, but not essential.
