We are seeking a highly skilled and motivated Community Nurse Specialist to join our team at Hywel Dda University Health Board. As a key member of our Community Team for People with a Learning Disability, you will play a vital role in delivering high-quality, evidence-based nursing care to adults and young people with learning disabilities.
Key Responsibilities- Provide specialist health care, treatment, and advice to a defined caseload of clients with learning disabilities and complex health needs.
- Liaise and work in partnership with other agencies to deliver a flexible and responsive service that meets the needs of the local population.
- Provide specialist advice, support, and training to other agencies responsible for providing services to people with learning disabilities.
- Registered Nurse Learning Disabilities (RNLD) with a current NMC registration.
- Post-registration training with evidence of ongoing professional development in the field of learning disabilities.
- Comprehensive understanding of current policy and practice in relation to people with learning disabilities.
- Teaching qualification or equivalent, or recent experience in the provision of training to health staff.
- Comprehensive understanding of local and national drivers for change in the field of learning disabilities.
- Extensive experience of working with people with learning disabilities at Band 5 or above.
- Experience in the assessment and management of people living with co-morbidities and complex health needs.
The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.
