About the Role
We are seeking a highly motivated and experienced Community Nurse to join our team at Hywel Dda University Health Board. As a Community Nurse Specialist, you will be responsible for providing high-quality care to patients in their own homes and the local community.
Key Responsibilities
- Deliver person-centered care to patients with a range of health needs
- Work collaboratively with other healthcare professionals to provide comprehensive care
- Develop and maintain strong relationships with patients, families, and carers
- Contribute to the development of community-based services and initiatives
- Participate in quality improvement activities to enhance patient outcomes
Requirements
- Registered Nurse (Adult) qualification and current NMC registration
- Experience in community nursing or a related field
- Ability to communicate effectively with patients, families, and colleagues
- Strong problem-solving and critical thinking skills
- Ability to work independently and as part of a team
Desirable Qualifications and Experience
- Post-registration experience in acute services or community nursing
- Teaching and assessing qualifications
- Community experience and knowledge of local health services
- Ability to speak Welsh (desirable)
