Job Summary

An exciting opportunity has arisen for a skilled and motivated nurse to join our Community Team for People with a Learning Disability in Hywel Dda University Health Board.

This role comes at a time of significant change in the healthcare sector, and we are seeking a professional who can contribute to the design and delivery of a new service model that provides flexible and responsive services to adults and young people with learning disabilities.

Main Responsibilities
  • To provide specialist health care, treatment, and advice to a defined caseload of clients with learning disabilities and complex health needs.
  • To work in partnership with other agencies to deliver a flexible and responsive service that meets the needs of the local population.
  • To provide specialist advice, support, and training to other agencies and healthcare professionals on a range of health issues affecting people with learning disabilities.

Requirements
  • Registered Nurse Learning Disabilities (RNLD) with a current NMC registration.
  • Post-registration training with evidence of ongoing professional development in the field of learning disabilities.
  • Comprehensive understanding of current policy and practice in relation to people with learning disabilities.
  • Teaching qualification or equivalent, or recent experience in the provision of training to health staff.
Desirable Qualifications and Experience
  • Comprehensive understanding of local and national drivers for change in the field of learning disabilities.
  • Extensive experience of working with people with learning disabilities at Band 5 or above.
  • Experience in the assessment and management of people living with co-morbidities and complex health needs.
Language Skills

We welcome applications from Welsh speakers, and the ability to communicate in Welsh is desirable for this post.
