The Lead Clinical and Care Strategist for Urgent Care plays a pivotal role in steering the Tameside Locality framework to fulfill the urgent care mandates outlined in the NHS Long Term Plan, ensuring the provision of safe and effective urgent care services for the community.

A primary objective of this position is to assist in the design and execution of services that enhance health and wellbeing, minimize the necessity for unplanned care, and support relevant care pathways.

This role is currently available as a Contract for Services opportunity, and candidates should be actively engaged with Greater Manchester GP's.

Please note that the existing contractual model is under review and may be subject to modifications.

Key Responsibilities

- Exhibit a commitment to collaborative work and serve as a clinical advocate for ongoing enhancements in urgent care; fostering established partnerships with stakeholders across GM ICS and within the Borough.

- Contribute clinical insights to the monthly Urgent Care Delivery Board and provide clinical leadership to the Urgent Care at Home initiative, proactively taking ownership of assigned tasks within agreed timelines.

- Inform and facilitate local execution of national directives, ensuring that colleagues in the locality are engaged and cognizant of their clinical obligations regarding such directives.

- Support initiatives aimed at managing urgent care demand and enhancing service quality for the population.

- Contribute to PCN developments that prioritize urgent and unplanned care needs.

- Collaborate effectively with the Tameside GP Alliance to mitigate unwarranted variations in access and services related to urgent care.

This role will challenge and provide clinical leadership for pathway advancements, ensuring a robust connection between primary care, community, and acute services.

About NHS Greater Manchester Integrated Care

Integrated Care Systems (ICSs) are collaborations of health and care organizations that unite to plan and deliver cohesive services, aiming to enhance the health and wellbeing of individuals residing and working in their area. Their mission is to improve outcomes in population health and healthcare; address disparities in outcomes, experiences, and access; boost productivity and value for money; and support broader social and economic development in their region. This will be achieved through neighborhood, place, combinations of places, and GM system initiatives.

Our NHS People Promise is a commitment we must all uphold to one another, working collectively to enhance the experience of working within the NHS for everyone.

Like many employers, NHS GM acknowledges the presence of unwarranted inequalities within the workforce, particularly concerning minoritized ethnic, female, and/or disabled staff and their intersectionality. Consequently, we will implement positive action measures to benefit our organization, including a broader pool of talented, skilled, and experienced individuals from which to recruit, as well as a deeper understanding of the needs of a more diverse range of clients.

Job Duties

1) Proactively collaborate across organizational boundaries to enhance health outcomes and the delivery of services throughout the urgent care system.

- Improve access to the most suitable urgent care services within the locality, ensuring that individuals with urgent care needs can reach the appropriate services promptly.

- Address health inequalities and gaps in service provision.

- Consider the needs of the local population.

2) Provide clinical leadership for the Urgent Care at Home initiative within the urgent care delivery framework of the Tameside Locality by working across interfaces with:

- Locality partners, particularly Primary Care contractors and ICFT regarding Digital Health, SDEC, etc., as well as other NHS providers, Council, and VCSE.

- NHS GM, including NHS GM Medical and Nursing Directorates, SCNs, and NHS GM System Boards.

3) Collaborate with the CCPL for Urgent Care Acute and GM focus.

4) Support the realization of the themes and priorities of the NHS GM Joint Forward Plan and national policies related to urgent care provision.
