Job Description

We are seeking a highly motivated and experienced Mental Health Care Professional to join our dedicated team. In this role, you will play a vital part in providing comprehensive support and care to individuals with a range of mental health conditions.

Responsibilities
  • Conduct thorough assessments of patients' needs and develop individualized care plans.
  • Implement and evaluate care plans, ensuring that patients receive the appropriate level of support and treatment.
  • Work collaboratively with multidisciplinary teams to coordinate patient care and facilitate seamless transitions between services.
  • Provide counseling, therapy, and other interventions to support patients' mental well-being.
  • Maintain accurate and up-to-date records of patient information and treatment progress.
Qualifications and Experience

The ideal candidate will possess:

  • A degree in health/social care or an equivalent qualification/experience.
  • Registration as a Mental Nurse, Occupational Therapist, Allied Health Professional, or Social Worker, enabling you to assess, plan, implement, and evaluate care for patients with mental health problems.
  • Substantial experience of managing a busy caseload or service users with diverse mental health conditions.
  • Proven ability to work effectively within multidisciplinary teams and multi-agency environments.

  • Excellent knowledge and understanding of the health and social care systems, clinical observations, and the Mental Health Act.
  • Strong interpersonal, influencing, communication, observation, and reporting skills.
  • Exceptional organizational skills and the ability to analyze situations and make sound decisions.
  • Empathy, support, and sensitivity towards the needs of others, coupled with the ability to cope with challenging circumstances.
  • The ability to thrive in a pressurized environment and apply practical problem-solving skills.
  • Competent IT skills and proficiency in navigating various systems and software packages (such as Outlook, databases, MS Office, and the internet).
  • Flexibility to work on a shift pattern and the ability to travel independently between locations.
  • The capacity to work effectively both collaboratively and independently, manage own workload, and supervise junior staff.
