About the Role

This is an exciting opportunity to join Berkshire Healthcare Foundation Trust as a Primary Care Mental Health Practitioner. As a key member of our team, you will be responsible for providing high-quality mental health services to patients in primary care settings.

Key Responsibilities
  • Provide mental health assessments and interventions to patients in primary care settings
  • Work collaboratively with GPs and other healthcare professionals to develop and implement care plans
  • Conduct mental state examinations and make onward referrals as necessary
  • Develop and maintain effective relationships with patients, families, and carers
  • Participate in multidisciplinary team meetings and contribute to the development of care plans
Requirements
  • Current professional registration as a Registered Mental Health Nurse, Social Worker, or Occupational Therapist
  • Minimum 5 years post-registration experience in working with people with mental health needs
  • Community experience in mental health services for adults and older adults with severe/significant mental illness
  • Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness
  • Considerable experience in the assessment and management of clinical risk
  • Experience of effective collaborative working in partnership to deliver holistic health and social care outcomes
