About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team in Primary Care. As a Primary Care Mental Health Practitioner, you will work closely with GPs and other healthcare professionals to provide high-quality mental health services to patients in primary care settings.

Key Responsibilities
  • Provide mental health assessments and interventions to patients in primary care settings
  • Work collaboratively with GPs and other healthcare professionals to develop and implement care plans
  • Conduct mental state examinations and make onward referrals as necessary
  • Liaise with secondary services and Talking Therapies to ensure seamless care coordination
  • Initiate first-line pharmacological treatments for Anxiety and Depression, as appropriate
Requirements
  • Current professional registration as a Registered Mental Health Nurse, Social Worker, or Occupational Therapist
  • Minimum 5 years post-registration experience in working with people with mental health needs
  • Community experience in mental health services for adults and older age adults with severe/significant mental illness
  • Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness
  • Considerable experience in the assessment and management of clinical risk
