About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a Primary Care Mental Health Practitioner, you will be working in a dynamic and supportive environment, providing high-quality care to patients with mental health needs.

Key Responsibilities
  • Provide mental health assessments and interventions to patients in primary care settings
  • Work collaboratively with GPs, nurses, and other healthcare professionals to deliver holistic care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct risk assessments and develop strategies to mitigate risks
  • Provide education and support to patients, families, and carers
Requirements
  • Current registration with the relevant professional body (e.g. NMC, HCPC)
  • Minimum 2 years' experience in mental health services
  • Experience of working in primary care settings
  • Advanced understanding of mental health needs and interventions
  • Excellent communication and interpersonal skills
