Job Description

As an Aftercare Coordinator with Home Group Limited, you will play a vital role in ensuring a smooth transition for our residents as they settle into their new homes.

Key Responsibilities:

  • Deliver high-quality aftercare services to residents, providing support and guidance to help them settle into their new environment.
  • Work closely with our development teams to ensure a seamless handover of properties, ensuring all necessary checks are completed and residents are fully supported.
  • Collaborate with our operations team to identify and address any issues that may arise, ensuring our residents receive the best possible service.
  • Develop and maintain strong relationships with residents, their families, and our partners to ensure a positive and supportive community.

What You'll Need:

  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
  • Proven experience in a similar role, with a strong understanding of the needs of vulnerable adults.
  • Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of our residents.
  • Strong problem-solving skills, with the ability to think creatively and develop innovative solutions.
