Position Overview

We are excited to announce a fantastic opportunity for a qualified Senior Clinical Practitioner to become a vital part of our community healthcare team. This role is essential in providing exceptional support within our healthcare services.

Role Requirements:

  • Completion of a Master's degree in Advanced Clinical Practice or equivalent advanced training.
  • Non-medical prescribing certification.
  • Demonstrated commitment to ongoing professional development.
  • Teaching qualification (998 or equivalent) is desirable.
  • Clear DBS certification.
  • Experience in managing acute, complex clinical presentations.
  • Proficient triage skills.
  • Strong understanding of autonomous practice in community healthcare, including strategies for preventing hospital admissions and facilitating safe discharges.

Key Responsibilities:

The successful candidate will undertake the following:

  • Assume accountability for clinical governance and leadership within the service.
  • Collaborate across various organizational and professional boundaries.
  • Establish effective partnerships in alignment with the strategic direction of the Trust and national initiatives.
