About the Role
We are seeking a highly skilled and dedicated Clinical Assessment Practitioner to join our experienced multi-professional service team at South West London and St George's Mental Health NHS Trust. The successful candidate will be responsible for assessing and managing caseloads of patients referred to our service, providing high-quality care and support to individuals with severe mental health illnesses.
Main Responsibilities
- Assess and manage caseloads of patients referred to our service
- Provide high-quality care and support to individuals with severe mental health illnesses
- Work collaboratively with multidisciplinary teams to develop and implement care plans
- Conduct thorough baseline mental health assessments and case formulation
- Develop and maintain effective relationships with patients, carers, and referrers

Person Specification
- Qualified social worker or occupational therapist with current relevant registration
- Practice Assessor qualification
- Excellent written and oral communication skills

- Ability to assess clinical situations quickly and use initiative
