Administrative Coordinator Opportunity for Long Term Conditions

This is a 6-month fixed-term secondment opportunity, with the possibility of an extension, for an Administrative Coordinator to work with early adopter sites and a small team to develop systems and processes to support remote review of High Risk and Complex patients for the PCN.

The Administrative Coordinator will work with a range of digital systems (EMIS/EPR) to follow-up, record, and communicate actions. This role will be suitable for a MDT Co-ordinator, Clinic Co-ordinator, or Pathway/Admin manager.

The Administrative Coordinator will report to the LTC Clinical Coordinator and will be responsible for developing and implementing administrative systems and processes to support the remote review of patients.

This role is offered as either a full-time (1 wte) position or part-time with a minimum of 0.5 wte. The remuneration for this post is at Agenda for Change, Band 5 or equivalent.

The Administrative Coordinator will work closely with the LTC Clinical Coordinator and the small team to develop and implement administrative systems and processes to support the remote review of patients.
