Position Overview

We are pleased to announce a remarkable opportunity to become a part of the Wound Care team as a Band 6 nurse within this specialized service. The ideal candidate will possess a minimum of three years of post-registration experience, along with prior exposure to wound care management and a solid understanding of pressure ulcer prevention protocols and pathways. This role encompasses working across various hospital sites and within the community, delivering expert assessment and treatment planning for patients with intricate wound care needs. Strong communication skills and the ability to collaborate effectively with professionals from diverse disciplines are essential for success in this position.

Key Responsibilities

- Collaborate with the Tissue Viability Nurse Specialist (TVNS) and multidisciplinary teams to provide a high standard of skilled care in the domain of wound management across all Trust settings.
- Offer specialized knowledge and guidance to colleagues and patients within a multidisciplinary framework.
- Deliver high-level clinical skills when providing direct patient care.
- Utilize evidence-based practices to assess, implement, and evaluate care programs tailored to meet patient needs.
- Adopt a holistic approach to patient and family care.
- Foster strong working relationships with nursing and allied health professionals for optimal patient outcomes.
- Monitor patient conditions and promptly report any significant changes or concerns to the TVNS or other healthcare professionals.
- Ensure that nursing interventions are clinically effective and grounded in evidence-based practices.

- Educate and support staff within the Trust regarding all aspects of wound care management.

Job Responsibilities

- Ensure adherence to Trust policies, procedures, and clinical guidelines.
- Promote equality, diversity, and patient rights.
- Continuously monitor care standards and contribute to quality improvement initiatives, including benchmarking, audits, and research.
- Maintain a database of clinical incidents related to pressure ulcers, identifying trends and training needs in collaboration with the TVNS.
- Identify and communicate opportunities for enhancing nursing practices, service development, and quality initiatives to the TVNS.
- Participate in staff meetings and working groups to identify areas for improvement and ensure that targets and objectives are met.
- Familiarize oneself with and comply with clinical governance initiatives in practice.
- Actively participate in audits and evaluations, adhering to Trust guidelines.

Professional Responsibilities

- Uphold confidentiality in accordance with the Data Protection Act, access to Health Records, and Consent for Treatment.
- Maintain active registration and revalidation with the professional register.
- Develop and maintain a professional portfolio.
- Adhere to the NMC professional code of conduct.
- Stay updated on skills, knowledge, and professional issues related to nursing and the specialty.
- Operate within Trust and national policies and standards.

Qualifications

Essential

  • 12 months of post-registration experience.
  • Registered Nurse with RN Adult branch.
  • Experience in managing patients with wound care needs.

Desirable

  • Academic study in Wound Care post-registration.
  • V300 non-medical prescribing qualification.
  • Experience in delivering training and education to staff groups.

Experience

  • Evidence of post-registration experience in wound care management.
  • Experience working autonomously.
  • Experience in multidisciplinary team collaboration.
  • Experience delivering care in various clinical environments.

Knowledge and Skills
