About the Role

Pilgrims Hospices is seeking a highly skilled and compassionate Registered General Nurse to join our team as a Palliative Care Specialist. As a key member of our multidisciplinary team, you will provide exceptional patient care and support to individuals and their families at the end of their life.

Key Responsibilities
  • Deliver high-quality palliative nursing care that reflects evidence-based practice and prioritizes patient choice and comfort.
  • Assess, plan, and implement individualized patient care plans, evaluating outcomes and making adjustments as needed.
  • Collaborate with internal and external healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Provide expert advice and guidance on symptom management and palliative care best practices.
  • Recognize and respond to changes in patient conditions, taking proactive steps to ensure patient safety and well-being.
  • Act as an independent prescriber, making informed decisions about patient care and treatment.
  • Contribute to service development and quality improvement initiatives, driving excellence in palliative care.
  • Supervise and educate other healthcare professionals, promoting knowledge sharing and skill development.
  • Participate in evidence-based projects and audits, informing best practices and policy development.
