About the Role
A challenging and rewarding opportunity has arisen for a skilled and experienced Practice Nurse to join our team at Ashford Medical Partnership. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.
Key Responsibilities
- Manage chronic disease, identifying significant abnormalities and developing individual management plans in collaboration with patients and other healthcare professionals.
- Conduct therapeutic monitoring, checking compliance with treatments and recognizing abnormalities to implement or recommend changes as necessary.
- Provide wound care and management services, assessing routine wounds and complex situations, and prescribing treatments as required.
- Perform patient health checks, identifying significant abnormalities and working with patients to develop management plans.
- Conduct risk assessments, recognizing issues and gathering information to refer patients as necessary.
- Undertake health screening, recognizing issues and gathering information to refer patients, and providing advice and monitoring services as required.
Requirements
- Registered General Nurse with a recognized qualification to diploma level or above.
- Minimum of 3 years' experience as a Practice Nurse, with evidence of continuing professional development activity.
- Proven ability to evaluate the safety and effectiveness of their own clinical practice.
- Skills in training and education, and past experience in management of Chronic Disease Management are desirable.
