About the Role

We are seeking a highly skilled and experienced Practice Nurse to join our team at Ashford Medical Partnership. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Chronic Disease Management
    • Identify significant abnormalities and develop individual management plans in collaboration with patients and other healthcare professionals.
    • Monitor and evaluate the effectiveness of treatment plans and make adjustments as necessary.
  • Therapeutic Monitoring
    • Check patient compliance with treatment plans and provide education and support as needed.
    • Recognize and respond to abnormalities and implement changes to treatment plans as necessary.
  • Wound Care and Management
    • Assess and manage routine wounds, including trauma.
    • Prescribe and implement wound care plans in complex situations, using techniques such as Doppler studies.
  • Patient Health Checks
    • Identify significant abnormalities and develop management plans in collaboration with patients and other healthcare professionals.
    • Conduct lifestyle questioning and identify health promotion issues.
  • Risk Assessment
    • Recognize and respond to issues that require referral, such as domestic violence, child protection, and vulnerable adults.
  • Health Screening
    • Recognize and respond to issues that require referral, such as women's and men's health, sexual health, and older people's health.
    • Conduct monitoring tasks, such as smears, and provide advice as necessary.
Requirements
  • Qualifications
    • Registered General Nurse with current registration with the NMC.
    • Qualifications in postnatal checks, diabetic reviews, smears, coils and ring pessaries, compression bandaging and wound care, Zoladex immunizations, ECG and BP measurements, vascular clinics, menopause, catheter care, Doppler studies, family planning, and sexual health.
    • Recognized qualification to diploma level or above.
  • Experience
    • Minimum of 3 years' experience as a Practice Nurse.
    • Evidence of continuing professional development activity.
    • Proven ability to evaluate the safety and effectiveness of their own clinical practice.
