Key Responsibilities:- Care Planning and Delivery: Assess, plan, implement, and evaluate care for patients with complex medical conditions, ensuring individualized care plans are developed and implemented in collaboration with interdisciplinary teams.
- Medication Administration: Administer medications and treatments in accordance with established protocols and standards of practice, maintaining accurate medical records and adhering to regulatory standards.
- Team Collaboration: Collaborate with interdisciplinary teams to develop and implement individualized care plans, ensuring effective communication and coordination of care.
- Patient Education: Provide education and support to patients and their families regarding their care and treatment plans, promoting patient empowerment and self-management.
- Quality Assurance: Monitor and document patient progress, responding to any changes in their condition, and adhering to regulatory standards and best practices.
- Qualifications: RGN (Registered General Nurse) qualification with current registration as an Adult Nurse, active UK NMC Pin, and proven experience in complex care settings.
- Skills: Strong communication and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams, and knowledge of relevant healthcare regulations and best practices.
