About the Role

The Ross Practice is seeking a dedicated and innovative Pharmaceutical Care Specialist to join our primary care network on a full-time basis. This role involves a variety of responsibilities aimed at enhancing patient care through expert pharmaceutical knowledge.

Main Responsibilities:
  • Conducting clinical assessments and providing treatment recommendations for patients, utilizing in-depth knowledge of medications for specific health conditions.
  • Offering specialized guidance on medication usage.
  • Performing comprehensive medication reviews and delivering health education to patients through various communication methods.
  • Leading initiatives focused on patient-centered medication optimization and quality enhancement.
  • Serving as a primary contact for all inquiries related to medications.
  • Facilitating improvements in clinical practices through audits and the implementation of necessary changes.
  • Managing clinics that focus on medication interventions, including anticoagulant monitoring.
  • Engaging in care planning for diverse patient needs, including mental health and end-of-life care.
  • Reviewing and acting upon daily pathology results.
  • Documenting relevant patient health information during consultations and contributing to quality outcome frameworks.
  • Monitoring patients with complex, chronic conditions to ensure optimal medication management.
  • Providing clinical expertise and advice regarding medications.
  • Building and maintaining professional relationships with patients.
  • Coordinating with other healthcare providers to ensure safe transitions of care.
  • Overseeing relevant components of the Directed Enhanced Service (DES) within the primary care network.
  • Taking an active role in shared-care protocols, clinical research involving medications, and collaboration with specialist pharmacists.
  • Managing the process for repeat prescription reauthorization, including reviewing requests and monitoring medication review dates.
  • Evaluating prescribing practices against local and national benchmarks, collaborating with the prescribing team to address any discrepancies.
  • Conducting structured medication optimization reviews in care facilities.
  • Contributing to efforts aimed at reducing medication waste across the primary care network.
About the Candidate

The ideal candidate will possess the following qualifications and skills:

  • A degree in pharmacy and registration with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy, whether in hospital, primary care, or community settings.
  • Qualification as an independent prescriber or actively pursuing this credential.
  • Membership in the Royal Pharmaceutical Society (RPS).
  • Familiarity with common acute and chronic conditions encountered in general medical practice.
  • Demonstrated pharmaceutical skills and knowledge in essential areas.
  • Ability to develop, manage, and review comprehensive care plans for patients with long-term conditions.
  • Accountability for delivering professional expertise and direct service provision.
  • Strong problem-solving skills with the ability to identify and address deviations from standard practices.
  • Adherence to legal, ethical, and professional standards and policies.
  • Support for patients in making informed decisions about their prescribed medications, in line with NICE guidelines.
  • A non-judgmental approach with flexibility and adaptability.
  • Self-motivated with exceptional communication abilities.
