Primary Care Careers is seeking a dedicated and innovative Pharmaceutical Care Specialist to join our dynamic primary care network. This full-time role involves working collaboratively within a supportive team environment, focusing on enhancing patient care through expert pharmaceutical knowledge.

Key Responsibilities

  • Conducting clinical evaluations and managing treatment plans for patients, utilizing extensive knowledge of pharmacotherapy for various health conditions.
  • Offering specialized guidance on medication management and therapeutic interventions.
  • Performing comprehensive medication reviews and providing health education to patients through various communication methods.
  • Leading initiatives aimed at optimizing medication use and improving healthcare quality.
  • Serving as a primary resource for medication-related inquiries.
  • Facilitating enhancements in clinical practices through systematic audits and the implementation of best practices.
  • Managing specialized clinics focused on medication interventions, including anticoagulation therapy.
  • Engaging in care coordination, particularly for mental health, preventive care, and end-of-life management.
  • Analyzing laboratory results and taking appropriate actions based on findings.
  • Documenting relevant patient health data during consultations, contributing to quality improvement frameworks.
  • Monitoring patients with complex, chronic conditions to ensure optimal medication management and adherence.
  • Providing expert clinical advice regarding pharmacotherapy.
  • Building and maintaining professional relationships with patients to support their healthcare journey.
  • Collaborating with other healthcare providers to ensure seamless transitions of care.
  • Overseeing relevant components of the Directed Enhanced Service (DES) within the network.

Qualifications and Experience

Essential Qualifications

  • A degree in pharmacy and registration with the relevant professional regulatory body.
  • Relevant post-qualification experience in pharmacy practice.
  • Qualification as an independent prescriber or in the process of obtaining this credential.

Essential Experience

  • Membership in a recognized pharmaceutical society.
  • Familiarity with common acute and chronic conditions encountered in primary care.
  • Demonstrated proficiency in core pharmaceutical skills and knowledge.
  • Ability to develop, implement, and review care plans for patients with long-term health issues.
  • Proven accountability in delivering professional pharmaceutical services.
  • Strong problem-solving skills with the ability to prioritize effectively.
  • Adherence to legal, ethical, and professional standards in practice.
  • Capacity to assist patients in understanding their medication regimens and promoting adherence.
  • A flexible and adaptable approach to patient care.
  • Self-motivated with excellent interpersonal communication skills.
