About the Role

We are seeking a highly skilled Rehabilitation Case Manager to join our team at Health and Care Management Ltd. As a Rehabilitation Case Manager, you will play a critical role in delivering high-quality clinical services to our clients.

The ideal candidate will have a background in sports therapy or sports rehabilitation and possess excellent communication skills, with the ability to influence and support patients throughout their recovery journey.

This is an exciting opportunity to utilize your clinical expertise in a telephonic, office-based environment, working with a team to provide effective case management services.

Key Responsibilities
  • Clinical Service Delivery: Deliver clinically robust, consistent, and comprehensive telephone assessments, leading to the most clinically appropriate outcomes.
  • Assessment and Planning: Assess, plan, implement, and evaluate appropriate interventions and clinical pathways throughout the case, leading to expedient patient recovery and case closure.
  • Patient Care: Provide high-quality patient care, including shared responsibility for patient cases, supporting colleagues, and adopting the highest standards of patient care.
  • Communication: Utilize excellent written and verbal communication skills to educate and provide clinical support to patients.
Requirements
  • Clinical Degree: Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy).
  • Experience: 1 year of clinical/hands-on postgraduate experience or previous case management or vocational rehabilitation expertise.
  • Registration: Current governing registrations of your relevant professional body (e.g., BASRaT or SST, HCPC body).
  • Skills: Excellent written and verbal communication skills, good attention to detail, and time management skills.
