About the Role

We are seeking a compassionate and skilled Health Care Assistant to join our team at Wirral Hospice St Johns. As a Health Care Assistant, you will play a vital role in providing high-quality care and support to patients with life-limiting illnesses in their own homes.

Key Responsibilities
  • Provide personal care and support to patients in their own homes, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with the hospice team to deliver a comprehensive care package, including nursing support, emotional support, and practical assistance.
  • Establish and maintain effective communication with patients, carers, and healthcare professionals to ensure seamless care delivery.
  • Recognize changes in patients' conditions and report to the appropriate healthcare professionals, ensuring timely interventions and support.
  • Participate in regular team meetings and safety huddles to ensure a safe and supportive environment for patients and staff.
Requirements
  • NVQ Level 2 in Health and Social Care or equivalent.
  • 6 months' experience as a Nursing Auxiliary or Health Care Assistant.
  • Ability to use a range of IT systems, including patient record systems.
  • Proven ability to provide personal care and support to patients with life-limiting illnesses.
