Job Summary

We are seeking a highly motivated and empathetic Health Care Assistant to join our team. As a Health Care Assistant, you will play a vital role in providing personalized care and support to patients in their own homes, enabling them to remain comfortable and dignified throughout their journey.

Key Responsibilities
  • Provide high-quality care and support to patients with palliative care needs, including personal care, medication management, and emotional support.
  • Work collaboratively with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Establish and maintain effective communication with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Recognize and respond to changes in patient condition, reporting concerns to registered practitioners as necessary.
  • Participate in ongoing training and professional development to enhance skills and knowledge in palliative care.
Requirements
  • NVQ Level 2 in Health and Social Care or equivalent.
  • 6 months experience in a care setting, preferably in palliative care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
