About the Role

We are seeking a compassionate and skilled Health Care Assistant to join our team at Wirral Hospice St Johns. As a Health Care Assistant, you will play a vital role in providing high-quality care and support to patients with life-limiting illnesses in their own homes.

Key Responsibilities
  • Provide personal care and support to patients in their own homes, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with the hospice team to deliver a seamless and comprehensive care service.
  • Establish and maintain effective communication with patients, carers, and healthcare professionals to ensure the best possible outcomes.
  • Recognize changes in patients' conditions and report to the appropriate healthcare professionals.
  • Participate in regular team meetings and safety huddles to ensure a safe and supportive environment.
Requirements
  • NVQ Level 2 in Health and Social Care or equivalent.
  • 6 months' experience as a Nursing Auxiliary/HCA or equivalent.
  • Ability to use a range of IT systems.
  • Proven ability to provide personal care.
  • Full driving license and access to a car for work purposes.
