Job Title: CPN - Adult Locality Team

We are seeking a skilled and compassionate CPN to join our client's Adult Locality Team in Cambridgeshire. As a key member of the team, you will play a crucial role in delivering high-quality community mental health services.

Key Responsibilities:
  • Participate in multi-disciplinary assessments of patients' health conditions
  • Formulate and implement care plans
  • Provide treatment, as necessary
  • Use evidence-based practice and specialist skills to assess, plan, implement, and evaluate interventions
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
  • Registered Mental Health Nurse with a valid NMC registration
  • Experience working in community mental health services
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
