Job Title: Community Nursing Specialist Practitioner

We are seeking an experienced Community Nursing Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our community nursing team, you will be responsible for providing high-quality nursing care to patients in their own homes and in various community settings.

The successful candidate will have a strong background in community nursing, with experience in leading and coordinating nursing teams. You will work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care and coordination.

This is an exciting opportunity to develop your clinical skills and expertise in community nursing, while contributing to the delivery of high-quality patient care. You will be part of a dynamic and supportive team, working in a fast-paced environment that requires flexibility and adaptability.

Main Responsibilities:
  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and in community settings.
  • Work closely with multidisciplinary teams to ensure effective patient care and coordination.
  • Develop and implement clinical skills and expertise in community nursing.
  • Contribute to the delivery of high-quality patient care, while maintaining a patient-centered approach.
Requirements:
  • Registered Nurse qualification.
  • Community Specialist Practitioner qualification.
  • Experience in leading and coordinating nursing teams.
  • Strong background in community nursing.
  • Ability to work in an integrated way with multidisciplinary teams.
