Community Health Specialist

Vacancies have arisen for Community Specialist Practitioners (CSPs) in south Manchester. As a CSP, you will work as part of our developing Neighbourhood team to lead and co-ordinate nursing teams to provide complex nursing care to adults in their own homes and in various community settings. You will work closely with the Multi-Disciplinary Team (MDT) to ensure effective patient care coordination, demonstrating a positive impact on our patients.
You will have the opportunity to develop new and existing clinical skills, as well as access to a range of training opportunities for personal development. To join us, you will need to demonstrate a keen interest in community nursing, together with a genuine desire to develop both the service and yourself. You will hold the relevant CSP qualification and have the required community nursing experience as outlined in the job description. You will be an excellent communicator and be willing to work in an integrated way with our partners in health and social care. With patient-centred care as your main focus, you will deliver our values and nurse with pride, respect, empathy, consideration, compassion, and dignity.
The Community Nursing Service operates 7 days a week with variable shift patterns available between 8 am-10 pm. Due to the nature of the role, it is essential that you possess a full driving licence and have access to a car. Main duties include working closely with the Band 7 team leader to ensure quality care delivery to patients in their own homes and residential settings.
