Job Summary

The Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a highly motivated and experienced nurse to join our Universal Crisis Team. As a Community Nurse, you will play a vital role in providing high-quality care and treatment to patients in their homes, working under the crisis model and pathway.

Main Responsibilities

Deliver care and treatment to patients with mental health crises, visiting them in their homes and providing various therapeutic interventions with ongoing risk management. Engage in daily multidisciplinary team reviews, liaise with other professionals, and work collaboratively as part of a closely knit team.

About the Role

This is an exciting opportunity for a skilled nurse to join our dynamic and enthusiastic team, who are HTAS accredited. You will be working in a challenging and rewarding environment, delivering high-quality care to patients in need.

Requirements
  • First Level Registered Nurse RNMH, RMN, RNLD or equivalent first level qualification
  • 12 months post-registration experience, and completed preceptorship
  • Well-developed inter-personal skills with ability to consult, liaise and negotiate with service users, carers, other professionals and wider community
