Job Summary

The Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a highly skilled and dedicated Community Nurse Practitioner to join our enhanced scaffolding provision team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care, working closely with individuals with complex needs, and ensuring the delivery of a safe, caring, responsive, and well-led service.

Main Responsibilities
  • Assess and manage a caseload of patients with substantial and complex needs, ensuring their safety and well-being.
  • Provide responsive and intensive specialist clinical advice to patients, families, carers, staff, and statutory agencies.
  • Develop and implement care plans, safety plans, and recovery and wellbeing plans, ensuring patients have access to regular reviews and adaptations as necessary.
  • Work collaboratively with the multidisciplinary team to deliver high-quality individualized care, promoting recovery and wellbeing, and maximizing independence.
  • Coordinate treatment planning, interventions, clinical reviews, and discharge planning, ensuring seamless transitions and optimal patient outcomes.

Person Specification
  • Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Mentor Training and evidence of post-registration training in clinical interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post, including training in CPA, Safeguarding, MAPPA, and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation and a proven ability to carry out comprehensive written assessments of individuals with complex health needs.
