About the Role

We are seeking a highly skilled and experienced Community Health Specialist to join our team in the Enhanced Scaffolding Provision. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care, working across the whole of Newcastle and Gateshead with individuals with complex needs.

Key Responsibilities
  • Assess and manage a caseload of patients with substantial and complex needs, who are at risk of hospital admission, placement breakdown, and presenting as acutely unwell.
  • Provide responsive and intensive specialist clinical advice to patients, families, carers, staff, statutory agencies, and third sector partner agencies.
  • Develop and implement care plans, safety plans, and recovery and wellbeing plans, ensuring patients have access to regular reviews to monitor progress and adapt plans as necessary.
  • Consider carer needs and ensure carers and family are involved in care planning.
  • Deliver evidence-based clinical interventions and coordinate treatment planning, interventions, clinical reviews, and discharge planning.
  • Provide clinical supervision to staff as delegated by Team Manager and Clinical Leads.
  • Provide compassionate care that is based on empathy, kindness, respect, and dignity.

Person Specification
  • Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Mentor Training.
  • Evidence of post-registration training in clinical interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
