About the Role

We are seeking a highly skilled and experienced Community Health Specialist to join our team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a Community Health Specialist, you will play a vital role in providing high-quality patient care and contributing to the development of our community services.

Key Responsibilities
  • Assess and manage a caseload of patients with complex health needs, providing comprehensive written assessments and developing individualized care plans.
  • Provide responsive and intensive specialist clinical advice to patients, families, carers, staff, and statutory agencies, ensuring safe and effective care.
  • Deliver evidence-based clinical interventions, including risk management, safety planning, and care and treatment reviews.
  • Work collaboratively with patients, families, and carers to develop care plans, safety plans, and recovery and wellbeing plans, ensuring patients have access to regular reviews and adaptations as necessary.
  • Coordinate treatment planning, interventions, clinical reviews, and discharge planning, ensuring seamless transitions and optimal patient outcomes.
  • Provide clinical supervision to staff as delegated by Team Manager and Clinical Leads, promoting professional development and excellence in practice.
Requirements
  • Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Mentor Training and evidence of post-registration training in clinical interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post, including experience of working in a community team and crisis management.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation and professional accountability, NMC standards, and other local and national initiatives.
