Job Summary

The Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a highly skilled and dedicated Community Nurse Practitioner to join our enhanced scaffolding provision. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care to individuals with complex needs, who are at risk of hospital admission, placement breakdown, and presenting as acutely unwell.

Main Responsibilities
  • Assess and manage a caseload of patients with substantial and complex needs, ensuring their safety and well-being.
  • Provide responsive and intensive specialist clinical advice to patients, families, carers, staff, statutory agencies, and the public.
  • Develop and implement care plans, safety plans, and recovery and wellbeing plans, ensuring patients have access to regular reviews and adaptations as necessary.
  • Collaborate with patients, carers, and families to ensure their needs are met and their voices are heard.
  • Deliver evidence-based clinical interventions and provide clinical supervision to staff as delegated by the Team Manager and Clinical Leads.

Person Specification
  • Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Mentor Training and evidence of post-registration training in clinical interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
