  • Older Adults Community Mental Health (OAMH) Team - 35 hours per week

Team Overview

ICMP East and West Teams

The ICMP provides a versatile service to all residents, aged 18 to 65, within the psychosis clusters who require secondary community mental health treatment and support.

This service emphasizes social inclusion, recovery, and self-management, offering individuals choice and involvement in their treatment plans. This approach leads to a personalized wellbeing plan that facilitates referrals back to primary care.

Older Adults Community Mental Health Team

This specialized integrated multidisciplinary team caters to adults over 65 experiencing severe and enduring mental health challenges or dementia. The team conducts health and social care assessments, offers support and advice for caregivers, and implements various interventions to enhance independent living and promote mental wellbeing.

Support is provided through care coordination and short-term psychological groups aimed at older individuals assessed for the service. All professionals collaborate with inpatient units and crisis teams to evaluate community support needs, contribute to multi-agency panels for care packages, and discuss safe discharges from hospital settings.

Key Responsibilities:

Social Workers are expected to manage caseloads effectively and work collaboratively with service users, their families, and caregivers to ensure a strength-based, person-centered approach is central to all interventions.

Be part of a multidisciplinary team that includes social workers, occupational therapists, psychiatrists, psychologists, support workers, lived experience practitioners, community psychiatric nurses, and administrative staff.

Work independently with individuals, caregivers, and families to deliver a variety of assessments and care interventions that promote mental health and personalized outcomes, while fostering independence. Responsibilities include:

  • Conducting social care assessments
  • Performing carer assessments
  • Managing court of protection cases
  • Addressing safeguarding concerns and inquiries
  • Executing mental capacity assessments
  • Conducting mental health assessments
  • Implementing Mental Health Act assessments

Senior social workers are expected to fulfill the role of an Approved Mental Health Professional (AMHP) on behalf of the Royal Borough of Greenwich.

Qualifications Required:

  • A registered professional social work qualification
  • An AMHP qualification
  • Registration with Social Work England
  • This role requires an Enhanced DBS check
