About the Role

We are seeking a dedicated and compassionate Community Practitioner to join our team at South London and Maudsley NHS Foundation Trust. As a Community Practitioner, you will play a vital role in providing high-quality mental health support to individuals in the Lambeth Borough.

Key Responsibilities
  • Manage a varied caseload in the Short-Term Support team, ensuring timely, safe, effective, and responsive service delivery.
  • Work collaboratively with the Multi-Disciplinary Team (MDT) to provide comprehensive care to service users.
  • Conduct clinical assessments and develop personalized care plans to meet the unique needs of each individual.
  • Provide education and support to service users, their families, and carers to promote mental health and well-being.
  • Participate in regular supervision and peer review to ensure high-quality practice and continuous professional development.
Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with significant relevant clinical experience.
  • Strong understanding of clinical governance, audit, and care coordination principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to provide quality care that is responsive to service users' needs, without close supervision.
  • Knowledge of clinical assessment, baseline mental health assessment, risk assessment, and crisis management.
