We are seeking a highly skilled and dedicated Community Mental Health Practitioner to join our team at Tavistock and Portman NHS Foundation Trust. As a key member of our Camden Adolescent Intensive Support Service (CAISS) team, you will play a vital role in supporting young people aged 11-18 years who are experiencing mental health crises in the community.
Key Responsibilities- Provide high-quality, person-centered care to young people and their families, working in partnership with other healthcare professionals and community agencies.
- Conduct comprehensive assessments and develop tailored care plans to address the complex needs of young people.
- Work collaboratively with the wider CAMHS team to ensure seamless transitions and continuity of care.
- Develop and maintain strong relationships with local organizations and community groups to enhance support networks and services for young people.
- Participate in regular team meetings and contribute to the development of the CAISS service.
- Registered Health Care Professional (Nurse, OT, SW, Psychology) with post-qualification experience in acute mental health settings.
- Degree-level qualification or equivalent.
- Excellent communication and interpersonal skills, with the ability to work effectively with young people, families, and multidisciplinary teams.
- Strong knowledge of relevant legislation, government policy, and guidelines.
- Ability to work independently and as part of a team, with a commitment to ongoing professional development.
