About the Role

We are seeking a highly motivated and experienced Mental Health Practitioner to join our team at South West London & St George's Mental Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for conducting mental health assessments, developing and implementing care plans, and providing support to individuals with dementia and their carers.

Key Responsibilities
  1. Conduct thorough mental health assessments and develop individualized care plans to meet the needs of clients and carers.
  2. Provide expert advice and support to clients and carers, recognizing factors that may affect their mental, emotional, and physical well-being.
  3. Act as a care coordinator, ensuring that all clients have care plans and that these are implemented and regularly reviewed.
  4. Develop and maintain a high standard of practice, using a range of psychosocial approaches within a health and psycho-social framework.
  5. Communicate effectively with relevant agencies to ensure an integrated programme of care is provided.
  6. Supervise and monitor the therapeutic effects and side-effects of prescribed medication, providing clear advice and information to clients.
  7. Organize and participate in the admission, discharge, and transfer of clients in accordance with Trust Policies.
  8. Provide clients and carers with up-to-date information on services offered by the Trust, how to express their views, and the complaints procedure.
  9. Develop and maintain effective working relationships with in-patient facilities regarding admission and discharge of clients.
  10. Facilitate carers assessments, provide care plans to meet identified needs, and monitor efficacy.
  11. Have knowledge and application of key legislation and policy, including the Mental Health Act, Care Programme Approach, Capacity Act, and Care Act.
  12. Participate in the running of the clozapine and depot clinics.
  13. Knowledge and experience of clinical assessment, risk assessment, MSE, mental capacity assessment, and crisis management.
  14. Good understanding of the Children's Act.
  15. Good verbal communication, written, and report writing skills.
  16. Knowledge of current Government Policy and relevant legislation concerning the provision of community mental health care.
  17. Understanding of safeguarding procedures.
Desirable Skills
  • Coaching.
  • Brief Solution Focused Therapy.
Requirements
  • Relevant post-qualification experience in Acute Mental Health.
  • Experience of providing thorough baseline mental health assessments and case formulation based on these.
  • Experience of working in a community multidisciplinary team setting and ability to undertake home visits/attend various clinical sites as required.
  • Experience of managing and monitoring budgets.
Education and Qualifications
  • Registered mental health nurse.
