About the Role

We are seeking a highly skilled and dedicated Community Mental Health Practitioner to join our team at Livewell Southwest. As a key member of our community-based mental health service, you will play a vital role in supporting the emotional, psychological, and social wellbeing of children and young people.

Key Responsibilities
  • Provide early intervention and support to children and young people with emerging mental health difficulties, promoting resilience and prevention of mental health issues.
  • Work closely with schools, community services, and other stakeholders to ensure timely assessments and effective interventions.
  • Coordinate and deliver evidence-based targeted interventions, including group-based needs-led groups and parenting programs.
  • Represent Livewell Southwest in multi-agency meetings, ensuring effective collaboration and coordination of mental health services.
  • Participate in clinical governance and quality improvement initiatives, contributing to the development of clinical policies and procedures.
  • Provide education and training to children, young people, and their families, enhancing their knowledge of mental health issues and management.
  • Liaise with other CAMHS pathways to ensure seamless transitions and effective care coordination.
Requirements
  • Strong clinical skills and experience in working with children and young people with mental health difficulties.
  • Ability to work effectively in a community-based setting, adapting to diverse environments and situations.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain accurate and detailed records, adhering to confidentiality and data protection policies.
