About the Role

We are seeking a highly skilled and compassionate Health and Wellbeing Coach to join our team at Fuller and Forbes Partnership. As a Health and Wellbeing Coach, you will play a vital role in supporting our patients to achieve their mental health and wellbeing goals.

Key Responsibilities

  • Provide individual support to patients to explore ways of improving their mental health and wellbeing
  • Support patients to utilise self-help and facilitated tools to aid individual goal planning and measurement of recovery
  • Potentially support patients to make informed choices about their health and wellbeing, including improving levels of physical activity, promoting a healthier diet, and improving mental wellbeing
  • Support access to wider community activities and services as part of an individual plan
  • Effectively signpost and support access to appropriate services
  • Support primary care colleagues to address the wider determinants of health and deliver individual packages of support and education
  • Facilitate collective action, support, and advice to people within local communities to reduce health inequalities while responding to identified health needs through one-to-one support
  • Utilise social prescribing to develop individual recovery and support planning, including working with the team directly across the practices
  • Promote effective and efficient working practices across the team to achieve measurable outcomes
  • Plan your own workload to promote and facilitate innovation and continuous improvement to deliver better services for service users and patients

Fuller and Forbes Partnership is a collection of 5 GP Surgeries based across Plymouth, providing healthcare to 35,000 patients. Our vision is to enable patients to live longer and healthier lives that are full, active, and meaningful. We aim to do this by developing a sustainable primary healthcare service, undertaken by our multidisciplinary team of GPs, Advanced Clinical Practitioners, Pharmacists, Paramedics, and Nursing teams, as well as our PCN staff.
