Job Title: Community Senior Mental Health Practitioner

We are seeking a highly skilled and experienced Community Senior Mental Health Practitioner to join our team at Livewell Southwest CIC. As a key member of our acute care pathway, you will work closely with established teams to provide timely assessments and support to individuals experiencing mental health crises.

About the Role

This is a challenging and rewarding role that requires a strong understanding of mental health issues and the ability to work effectively in a fast-paced environment. As a Community Senior Mental Health Practitioner, you will be responsible for:

  • Providing clinical risk assessments and contingency planning for individuals presenting in mental health crisis
  • Working closely with other mental health services and agencies to provide face-to-face assessments and support
  • Empowering patients, carers, and their families in decision-making and diverting to health and social care resources for support
  • Leading on investigations for complaints, complex cases, and internal reviews with management support where required
Key Responsibilities

As a Community Senior Mental Health Practitioner, you will be responsible for:

  • Acting as shift lead on a rota basis, coordinating service delivery, prioritizing workload, and providing clinical advice to junior staff
  • Offering consultation to other services and contributing to a multi-agency forum
  • Contributing to the training and supervision of others
  • Participating in and chairing Risk Management and Professionals meetings as required
Requirements

To be successful in this role, you will need:

  • A degree in Healthcare in a relevant area supplemented by a post-graduate qualification or specialist training with relevant experience
  • Evidence of continuing professional development
  • Experience of working in mental health care/treatment relevant to the service
  • Experience of working with people presenting in mental health crisis
  • A clear willingness to develop skills in the areas of both functional mental health problems and older people's/complex care mental health needs
