About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Older Peoples Mental Health (OPMH) Functional Team based at the Beauchamp Centre in Mount Gould Hospital, Plymouth. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals aged 65 and above with functional mental health conditions.

Key Responsibilities
  • Assess new referrals and provide nursing care and interventions as appropriate.
  • Act as a keyworker for an allocated group of patients and organize care in accordance with the Personalised Care Framework.
  • Liaise with other professionals, including social services, police, and healthcare providers, to ensure seamless care coordination.
  • Keep accurate and up-to-date electronic and paper records of patient and staff activity, adhering to NMC and HPC guidelines.
  • Participate in mandatory training and developments in mental health nursing or sphere of practice.
Requirements
  • A registered mental health or learning disabilities nurse or a final year student mental health nurse with previous experience in working in a mental health team.
  • Knowledge of the Mental Health Act, Personalised Care Framework, and NICE guidelines.
  • Ability to travel across Plymouth and possess a full driving license for at least 12 months.
