About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our First Response Service at Livewell Southwest. As a key member of our team, you will play a vital role in providing high-quality mental health care and support to individuals in crisis.

Key Responsibilities
  • Provide brief clinical risk and mental health assessment, formulation, and short-term contingency planning for individuals presenting in mental health crisis.
  • Work collaboratively with key partner organizations, service users, carers, clinicians, and other practitioners within a multi-disciplinary team to deliver services.
  • Develop and implement effective interventions to empower patients, carers, and their families in decision-making and divert to a range of health and social care resources for support.
  • Work in a range of settings, including primary care, community settings, and other health service sites, as required.
  • Collaborate with multi-agency colleagues, including police and paramedics, to provide acute assessment and care and support to individuals experiencing mental health crisis in the community.
Requirements
  • Relevant degree in healthcare, supplemented by a post-graduate qualification or specialist training with relevant experience.
  • Professional qualification (RMN, Social Worker, Occupational Therapist) and professional registration (NMC, HCPC, Social Work England).
  • Evidence of continuing professional development and mentorship qualification or equivalent experience.
  • Ability to work collaboratively with key partner organizations and service users, carers, clinicians, and other practitioners within a multi-disciplinary team.
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
