Discharge Support Worker

We are seeking a highly motivated and empathetic Discharge Support Worker to join our team at Livewell Southwest. As a Discharge Support Worker, you will play a vital role in supporting individuals with mental health needs to achieve their recovery goals and maintain their independence in the community.
Key Responsibilities- Work as part of a multidisciplinary team to deliver physical, social, and psychological care to individuals with mental health illness.
- Provide support to individuals with complex needs, promoting their recovery and maintaining them in their community environment.
- Assist the multidisciplinary team in assessing, planning, implementing, and evaluating care, and implementing discharge plans.
- Deliver physical, social, and psychological care to individuals with mental health difficulties, working in hospital and community settings.
- Build therapeutic relationships with individuals, their relatives, and carers, providing practical and emotional support to aid recovery and facilitate discharge from hospital.
- Document relevant information in multidisciplinary records using the Care Programme Approach (CPA) process.
- Work in accordance with the Loan Workers Policy and maintain confidentiality in respect of individuals at all times.
- Motivated and empathetic individual with a passion for supporting individuals with mental health needs.
- Effective communication and interpersonal skills, with the ability to build relationships with individuals, their relatives, and carers.
- Ability to work in stressful conditions and as part of a team.
- Knowledge of mental health services, including the Care Programme Approach and the Mental Health Act.
- Experience of working in a variety of mental health settings, including acute and community services.
- Ability to support individuals in community settings and provide practical assistance with daily living skills.
