About the Role

Livewell Southwest is seeking a highly skilled Discharge Coordinator to join our team in Glenbourne Unit / Recovery services. As a Discharge Coordinator, you will play a crucial role in supporting individuals with complex health and social needs to achieve a smooth transition from in-patient care to community-based services.

Key Responsibilities

  • Develop and implement discharge plans in collaboration with multidisciplinary teams, patients, and their families.
  • Identify and address social, housing, and financial barriers to discharge, ensuring timely and effective resolution.
  • Work closely with Adult Social Care, Occupational Therapy, and other agencies to ensure comprehensive support for patients.
  • Conduct regular reviews and updates of discharge plans, ensuring patient needs are met and goals are achieved.
  • Collaborate with the Discharge Team Leader and wider multidisciplinary team to ensure seamless communication and coordination.
  • Manage databases and maintain accurate records of patient information, discharge plans, and progress.

Requirements

  • NVQ Level 5 or Foundation Degree in a relevant field, or equivalent experience and skills.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Knowledge of the Acute Care Pathway, local providers, and community links.
  • Understanding of the Mental Health Act and safeguarding processes.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
