Job Title: Discharge Coordinator

Job Summary:

Livewell Southwest is seeking a highly skilled and compassionate Discharge Coordinator to join our team. As a key member of our Discharge Team, you will play a vital role in ensuring the smooth transition of patients from our in-patient services to community-based care.

Key Responsibilities:

  • Develop and implement discharge plans that meet the individual needs of patients, taking into account their physical, emotional, and social requirements.
  • Work closely with multidisciplinary teams, including healthcare professionals, social workers, and community services, to ensure seamless communication and coordination.
  • Identify and address potential barriers to discharge, such as social, housing, and financial issues, and develop strategies to overcome these challenges.
  • Monitor patient progress and adjust discharge plans as needed to ensure timely and effective transitions.
  • Collaborate with patients, families, and carers to empower them to take an active role in their discharge planning and recovery.

Requirements:

  • Relevant experience working with individuals with mental health needs and a strong understanding of the discharge process.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Knowledge of local health and social care services, including community resources and support services.
  • Ability to maintain accurate and up-to-date records, including patient notes and discharge plans.
